Physiology

Both serotonin and norepinephrine influence mood, energy, and pain perception.

Mechanism of Action

Inhibit reuptake of both serotonin and norepinephrine, increasing their synaptic availability.

Indications

  • Major depressive disorder
  • Generalized anxiety disorder
  • Neuropathic pain (duloxetine)
  • Fibromyalgia

Side Effects / Adverse Effects

  • Nausea, dry mouth
  • Hypertension (dose-related, especially venlafaxine)
  • Sexual dysfunction
  • Insomnia
  • Sweating
  • Discontinuation syndrome

Contraindications

  • Concurrent MAOI use
  • Uncontrolled hypertension
  • Narrow-angle glaucoma

Nursing Considerations

  • Monitor blood pressure regularly (especially venlafaxine)
  • Takes 4–6 weeks for full effect
  • Taper slowly to avoid withdrawal symptoms
  • Monitor for suicidal ideation
  • Avoid alcohol and other CNS depressants
