Physiology

Prostaglandins protect the gastric mucosa by stimulating mucus and bicarbonate production and reducing acid secretion. NSAIDs reduce prostaglandins, increasing ulcer risk.

Mechanism of Action

They mimic prostaglandins, increasing mucus and bicarbonate secretion and decreasing gastric acid production.

Indications

  • Prevention of NSAID-induced ulcers

Side Effects / Adverse Effects

  • Diarrhea (most common)
  • Abdominal cramping
  • Uterine contractions (can cause miscarriage)

Contraindications

  • Pregnancy (causes uterine contractions / abortion)

Nursing Considerations

  • Pregnancy test before starting in women of childbearing age (critical safety issue)
  • Educate women to use contraception (teratogenic risk)
  • Take with food (reduces diarrhea)
  • Monitor for diarrhea and cramping
  • Used in patients on long-term NSAIDs
