Physiology

The pancreas secretes enzymes (lipase, amylase, protease) to digest fats, carbs, and proteins. In pancreatic insufficiency (cystic fibrosis, chronic pancreatitis), these enzymes are deficient.

Mechanism of Action

They replace deficient pancreatic enzymes, allowing normal digestion and absorption of nutrients.

Indications

  • Cystic fibrosis
  • Chronic pancreatitis
  • Pancreatic insufficiency
  • Post-pancreatectomy

Side Effects / Adverse Effects

  • Abdominal discomfort
  • Nausea
  • Allergic reactions (pork-derived)
  • Fibrosing colonopathy (high doses, especially in CF)

Contraindications

  • Pork protein allergy
  • Acute pancreatitis (early stage)

Nursing Considerations

  • Give with every meal and snack (mimics natural enzyme release)
  • Do not crush enteric-coated capsules (acid would destroy enzymes)
  • Wipe lips after dosing (enzymes can irritate skin/mouth)
  • Monitor weight and stool (steatorrhea improvement = effectiveness)
  • Take with H2 blocker or PPI if prescribed (protects enzymes from gastric acid)
