Physiology

Aldosterone from the adrenal cortex regulates sodium retention, potassium excretion, and blood volume.

Mechanism of Action

Synthetic aldosterone β€” promotes sodium and water retention and potassium excretion at the renal tubules.

Indications

  • Adrenal insufficiency (Addison's disease)
  • Orthostatic hypotension (off-label)
  • Salt-wasting congenital adrenal hyperplasia

Side Effects / Adverse Effects

  • Hypertension
  • Edema, weight gain
  • Hypokalemia
  • Heart failure (volume overload)
  • Headache

Contraindications

  • Heart failure
  • Uncontrolled hypertension
  • Severe edema

Nursing Considerations

  • Monitor BP, weight, and edema regularly
  • Monitor potassium levels (hypokalemia risk)
  • Encourage potassium-rich foods if levels low
  • Daily weights β€” report rapid gain
  • Often given with hydrocortisone in Addison's disease
  • Teach signs of fluid overload
