Physiology

Bacterial protein synthesis depends on the 30S ribosomal subunit.

Mechanism of Action

They bind the 30S ribosomal subunit, blocking bacterial protein synthesis (bacteriostatic).

Indications

  • Acne
  • Lyme disease
  • Chlamydia
  • Rickettsial infections
  • Atypical pneumonia

Side Effects / Adverse Effects

  • Photosensitivity (severe sunburn)
  • Tooth discoloration in children
  • GI upset
  • Hepatotoxicity
  • Esophageal irritation

Contraindications

  • Pregnancy and breastfeeding
  • Children under 8 years (tooth staining, bone growth issues)

Nursing Considerations

  • Avoid in pregnancy and children under 8 (tooth/bone effects)
  • Take with full glass of water and remain upright 30 min (prevents esophageal ulcers)
  • Avoid dairy, antacids, iron, calcium within 2 hours (chelation reduces absorption)
  • Use sunscreen and protective clothing (photosensitivity)
  • Do not use expired tetracyclines (nephrotoxic when degraded)
